* Team Registration (Must be RECEIVED by Cut Off date above!) Envelope must be postmarked by the appropriate date above in order to receive the Early Bird registration discount.

1. Dodgeball, Kickball (Indoor/Outdoor), Softball, and Recess Team fees each cover up to 13 players; add $40 for each additional player
2. Volleyball Team fee covers up to 10 players; add $50 for each additional player
3. Bowling Team fee covers up to 6 players.
4. Soccer Team fee covers up to 18 players.

Bounced checks are subject to a $25 service charge up to treble the amount of the check pursuant to California Civic Code 1719.
